LfA Summer Playschemes 2011

We were very successful in running 5 schemes again this year. The dates were as follows:

Parents

If you wish to apply for a place on next year's schemes then please complete both the Application Form (PDF) and Choices and Preferences Form (PDF). Both forms should be returned to 69 Fairlands Road, Sale, Cheshire M33 4AU with a stamped addressed envelope, ensuring that you use the correct postage (you will need to affix a "Large" stamp when you return the application form). The envelope will be used to notify you of your allocations and will include a PECS strip showing the week's activities. If the incorrect postage is paid then the Post Office will not deliver the envelope and we will not have capacity to pay either the excess or collect the envelopes.

Staff

If you have worked for us before then there is no need at the moment to do anything. We will contact you again in 2012 with more details.

If you have never worked for us before then please complete the Staff full Application Form.pdf (PDF). It is important that you complete the correct form otherwise your application will be delayed.
